Welcome! We’re so glad you’re here. Being approved for Support Coordination is an important step, and at A Simple Leap Support Services, our role is to walk alongside you—making sure you feel supported, informed, and empowered every step of the way.
Here’s what you can expect next.
Meet Your Support Coordinator
You will be assigned a Support Coordinator (SC) who will be your main point of contact. Your SC will:
Get to know you, your goals, and what matters most to you
Explain your DDD program, tier, and service options
Help you understand your rights and choices
Your Support Coordinator works for you.
Planning Your Services (ISP & PCPT)
Together, we will review or complete your:
PCPT (Person-Centered Planning Tool), which captures your goals, preferences, and support needs
ISP (Individualized Service Plan), the official plan that authorizes services
This plan is built with you, not for you.
Choosing Providers & Starting Services
Your Support Coordinator will help you:
Explore available service providers
Schedule meet-and-greets if needed
Begin services within 90 days of ISP approval (this is a DDD requirement)
Starting services on time helps keep everything moving smoothly.
Monthly Check-Ins & Ongoing Support
Once services begin:
Your Support Coordinator will contact you every month
Contacts may be in person, by phone, or virtual
These check-ins help make sure services are working and needs are being met
You can always reach out between monthly contacts if something changes.
Monitoring, Reviews & Updates
Your Support Coordinator monitors services throughout the year
Quarterly reviews help ensure services remain appropriate
An annual ISP review updates goals, services, and budgets
Plans can be updated sooner if your needs change
Important Documents to Keep Handy
You may be asked for documents such as:
Medicaid information
Identification
Guardianship paperwork (if applicable)
Medical or evaluation records
Having documents ready helps prevent delays.
Guardianship & Decision-Making
If you are your own decision-maker, you will sign your ISP
If you have a legal guardian, the guardian signs on your behalf
A guardian is a person legally appointed to help make decisions when needed.
