A Simple Leap is seeking to hire professionals to assist individuals and their families in the development of Individualized service plans (ISP) to address their service needs by identifying and connect them with needed services and supports to improve their quality of life within the home and the community.

This is a P/T Position with a F/T Potential

We are currently seeking a support coordinator with the following:

At least two years' experience as a Support Coordinator (NJ DDD)

Bilingual (Spanish/English) a plus

Able to service participants in the North and Central NJ areas

Very Knowledgeable with iRecord data base

Communicates well with good writing skills

Must have references

Able to carry a minimum of 10 cases

Own car/clean record

A Bachelor's Degree

Availability during the days for meetings
